https://t.co/PBsUaVnn8u

Abstract in brief
- The traditional view of a simple negative feedback loop between the thyroid and pituitary glands is outdated; recent studies show the relationship is more complex, involving multiple feedback mechanisms, dynamic interactions, and a proposed feedforward system, which make the regulation of thyroid hormones more situation-dependent and mathematically intricate.
- TSH (thyroid-stimulating hormone) is not a fixed marker for normal thyroid function (euthyroidism), especially during therapy with levothyroxine (lT4), where TSH levels may not accurately reflect tissue thyroid hormone status, particularly the levels of active T3 (triiodothyronine), leading to potential tissue-level deficiencies despite normal serum TSH.
- TSH also plays a vital role beyond T4 regulation, directly influencing deiodinase enzymes that convert T4 to T3, thereby helping maintain T3 balance at the tissue level; however, lT4 therapy can disturb this balance, often resulting in lower tissue T3—even when blood TSH is normal—highlighting the need to reassess how thyroid hormone regulation and treatment effects are understood.
