Stiffness of the iliotibial band and associated muscles in runner's knee: Assessing the effects of physiotherapy through ultrasound shear wave elastography.
Discussions highlight skepticism about the effectiveness of stretching, foam rolling, and strength training for ITB issues, with comments like 'weak study' and debates on whether stiffness increases following therapy. The tone varies from critical to analytical, reflecting a nuanced engagement with the article's findings and their implications for common treatment practices.
AgreementNeither agree nor disagree
Many discussions acknowledge that the study questions common treatments like stretching and foam rolling, which reflects neutrality or slight disagreement with traditional views.
InterestModerate interest
Posts show moderate curiosity, especially when debating whether ITB stiffness is a key factor or when referencing the implications for physiotherapy practices.
EngagementModerate engagement
Several posts analyze the study's methodology, implications, and compare it to other research, indicating a moderate level of engagement.
ImpactNeutral impact
The discussions suggest some influence on treatment approaches, but overall, the content seems to cause more debate than immediate change, resulting in a neutral impact score.
Abstract in brief
The study tested whether Iliotibial Band Syndrome (ITBS) is caused by increased tension in the iliotibial band due to weak hip muscles and whether physiotherapy can reduce ITB stiffness.
The research involved 14 runners with ITBS and 14 healthy controls, measuring hip muscle strength, pain levels, and ITB stiffness before and after 6 weeks of physiotherapy.
Results showed no significant difference in ITB tension between affected and healthy legs, and although hip muscle strength improved with physiotherapy, ITB stiffness actually increased, suggesting that ITB tension may not be the main cause of ITBS and that current treatments may not effectively reduce ITB tone.

People will say stretching and foam rolling is not evidence based and garbage care for ITB pain and then cite a weak study or opinion for some other intervention.
Well, here is another weak study that supports stretching and foam rolling

Increases in ITB stiffness following a stretching and strengthening program (you know just variations on tensile loading).

...in any connective tissue. There is nothing special about the ITB. In fact, I'd suggest that the tensile load (strain) is actually more likely to catalyse increases in stiffness. See Friede (https://t.co/k4EuyAzmiG) or consider what happens to a tendon with strength training

Much like stretching and strengthening was part of a global approach as well in Fredericson's original studies. There hasn't been much since but strength training is routinely considered best practice

@KWong_PTPhD IMO, the long term response to tensile strain (e.g stretch) is to actually stiffen the tissue. Same thing that happens in tendons with strength training. Resistance exercises cause strain (i.e stretch) and the tissue responds by increasing stiffness. https://t.co/k4EuyAzmiG
