The European Society of Gastrointestinal Endoscopy (ESGE) recommends taking multiple biopsies from different segments of the colon, especially in patients with signs of colitis, with specific guidance for collecting samples in suspected microscopic colitis and in inflammatory bowel disease surveillance, emphasizing targeted and systematic approaches.
For high-risk patients, such as those with a history of colonic neoplasia or ongoing inflammation, ESGE suggests combining chromoendoscopy with targeted biopsies and additional non-targeted biopsies every 10 cm along the colon, although this is supported by low-quality evidence.
In cases of pouch surveillance or ulcerative colitis with signs of inflammation, ESGE advises systematic biopsies from affected areas and specific regions like the ileal loop and anorectal cuff, to accurately assess dysplasia or disease activity, despite the recommendations being based on low to moderate quality evidence.]
