The discourse centers on the STEP 1 extension findings that two-thirds of lost weight returned within a year after withdrawal, with cardiometabolic gains waning. Respondents range from neutral observers to critical voices arguing for lifelong treatment or lifestyle changes, with many citing the paper to push for sustained management of obesity. The language spans neutral summaries, critiques, questions, and advocacy, reflecting a broad, somewhat skeptical but highly engaged discussion about the implications of the study for obesity treatment and health policy.

Abstract in brief
The STEP 1 trial assessed the effects of subcutaneous semaglutide on weight loss and cardiometabolic risk factors in adults with obesity over 68 weeks, followed by a one-year treatment withdrawal phase.
Participants experienced significant weight loss with semaglutide, averaging a 17.3% reduction, while those on placebo lost only 2.0%, but most regained a substantial portion of this weight after stopping treatment.
Results indicated that cardiometabolic health improvements also declined after treatment ended, suggesting long-term care is essential for sustaining weight loss and health benefits.
A recent study examines what happens when the new weight loss drug semaglutide (Wegovy) is withdrawn after 68 weeks of treatment. Some interesting things to chew on in this paper.

@semodough This is the real problem in obesity care. In the STEP 1 extension, people regained about two-thirds of their lost weight within a year of stopping semaglutide (Wilding 2022, DOI 10.1111/dom.14725). Anything that helps people hold the loss after GLP-1s is worth watching.

That may be because after a year of steady weight gain - in which subjects regained on average 66% of weight lost - they quit measuring. There's no reason to think that the weight regain stopped when the study did.

@DownedUAP @DrNadolsky Most regain some (but not all) of the weight they loss after they quit: https://t.co/TW3WhrqYIH. So there's still a net benefit for most after quitting.
Loss in muscle is not induced by the drug but weight loss in general when you don't also lift weights.

@DrEenfeldt The pattern of weight gain in weight-reduced vs. non-weight-reduced people can only be explained by invoking a homeostatic reaction in weight-reduced people. They gain weight much faster than non-weight-reduced people.

One of the downsides isn't a side effect, but more related to how the drugs work. Using them for weight loss is already being shown to be effective only for as long as you take them, which comes with the implication of a lifelong commitment.

@DobryJane @MedscapeEndo In case you have an interest in facts, all the weight does not "come back plus more." The fact is that people only regain 2/3 of the weight they lost a year after they stop taking it. Obesity is a chronic condition. Here is the study:
