The ESGE guidelines emphasize that superficial gastrointestinal lesions should be evaluated using high-definition endoscopy combined with chromoendoscopy, but routine use of advanced imaging like EUS, CT, MRI, or PET-CT is not recommended before endoscopic resection.
Endoscopic submucosal dissection (ESD) is recommended as the preferred treatment for most superficial esophageal, gastric, and certain Barrett's esophagus-related lesions, especially when there's suspicion of submucosal invasion or for larger, fibrotic, or difficult-to-remove lesions.
Achieving an en bloc R0 resection (complete removal with clear margins) in superficial gastrointestinal lesions, particularly in cases of intramucosal cancer or well-differentiated tumors without lymphovascular invasion, is considered curative with minimal further treatment needed.]
