Abstract in brief
Sodium-glucose cotransporter 2 (SGLT-2) inhibitors, like dapagliflozin, are thought to enhance heart health mainly due to their ability to promote water loss (osmotic diuresis) through increased glucose in urine.
The DAPA-Shuttle trial examined whether the body's response (vasopressin levels) to conserve water could cancel out the fluid loss effects of glucosuria from dapagliflozin in heart failure patients.
Results showed that while dapagliflozin significantly increased glucose in urine and raised copeptin levels (a marker related to water retention), it did not meaningfully increase total urine volume, indicating that vasopressin may indeed mitigate its diuretic benefits
#SGLT2i release ⬆️ glucose into the urine, & thereby have the potential to pull ⬆️ fluid from the body via osmosis into the urine & decongest pts w/ heart disease
New data, however, casts doubt on this presumed diuretic action of SGLT2i in pts w/ HF https://t.co/Lac611HC6X #JACC https://t.co/Zi02J3kbmA

Amazing study showing SGLT2i do NOT cause osmotic diuresis.
Increased renal solute is offset by vasopressin mediated concentrating of urine = no change in urine volume
Translation: it’s likely unnecessary to hold SGLT2i for rising Cr 2/2 hypovolemia

Interesting @JACCJournals article about possible mechanism of #SGLT2i benefit in HF.
Perhaps the benefit is not in osmotic diuresis, but instead "“nutrient deprivation signaling/autophagy hypothesis” via "reprogramming of mitochondrial function."
