Abstract in brief
The study tested whether adding niraparib, a PARP inhibitor, to standard treatment with abiraterone acetate and prednisone (AAP) could improve outcomes for patients with metastatic castration-sensitive prostate cancer (mCSPC) who have HRR gene alterations, especially within 6 months of hormone therapy start.
Results showed that in patients with BRCA1/BRCA2 or other HRR gene alterations, this combination significantly extended radiographic progression-free survival (median not reached versus 26 months for AAP alone), with a hazard ratio of 0.52 in the BRCA subgroup and 0.63 overall, indicating a reduction in progression risk.
However, the combination also increased adverse events, with 75% of patients experiencing grade 3 or 4 side effects (mainly anemia and hypertension), and there were 14 treatment-related deaths in the niraparib group compared to 7 in the placebo group, highlighting the balance between potential benefits and risks.]
