We self diagnose everything, doctors or specialists can't know if you're in pain, if you have a headache, if you have mental health issues, if you have tinnitus, the list goes on. There're very, very few things can be diagnosed without you saying "hey doc I'm...."
You might need to add an audiologist who can provide sound therapy and tinnitus retraining therapy. And perhaps utilize an otolaryngologist from a medical university. Teaching doctors are usually more keen on searching for the obscure.
I think if someone did this, at best, the mocking laughter would be a form of tinnitus that follows them the rest of their life

Tinnitus is insane this morning. On top of the high frequency noise I've had since my accident I now have what I think is 'glue ear' and it feels like someone is blowing a deep vuvuzela down my right ear, which is almost totally non-functioning.
Lastly, I wanna include a bit of hope because this is so awful and I wanna try to help if I can. I have tinnitus too and there's a bit of a trick you can do to dull the sound. When you mentioned hitting your head and it flaring up before going down for a bit, I knew you inadvertently touched on it.

I lost my high upper range hearing in the same 2008 vehicle accident that gave me tinnitus. So at least in 1 ear (somehow I fixed the other) I now hear the mosquito noise all the time, basically. But at least it doesn’t jump scare me anymore in public places. You can get used to anything I guess.

I work for a good company & have pretty good insurance but have 0% coverage on hearing aids. 0% on a hearing test. What the fuck! I’m grateful that these still work for me. I hope you can find something that works for you. If you buy over-the-counter, make sure they work for tinnitus.

I went to a place about 10 years ago called sound relief, they’re all over Colorado. They specialize in tinnitus, got some hearing aids that play barely Audible light Zen music when I wear them. It doesn’t interfere with conversations or hearing. That trains my brain not to hear the ringing.
