Abstract in brief

Catheter ablation for persistent atrial fibrillation is less effective than for paroxysmal types, and adding substrate modifications like complex electrogram ablation or linear ablation does not significantly improve outcomes compared to pulmonary vein isolation alone.
In a study of 589 patients followed for 18 months, 59% of those with only pulmonary vein isolation remained free of AF, versus 49% with complex electrogram ablation and 46% with linear ablation; procedure times were shorter for the PVI-only group.
Complication rates included tamponade, stroke, transient ischemic attack, and a rare atrioesophageal fistula, with no significant differences among the different ablation strategies in secondary outcomes like freedom from atrial arrhythmias after multiple procedures.
