Cochrane’s Editor in Chief has received a revised version of the review on exercise therapy for #MECFS. A revised report will be published within 2 months. I’m afraid that the main problem—subjective outcomes in an unblinded trial—will not be addressed.

When I was first diagnosed with Long Covid, I came across this @bmj.com article written by...er, Paul Garner emphasising pacing & highlighting the disbelief those with ME/CFS have faced for decades :

Luckily this enabled me to find useful resources on pacing etc.

This is too important not to have ALT.
What Cochrane is doing here to people with ME/CFS is anti-science and criminal. Never before ME/CFS could I have imagined how ideological science is.
Cochrane has decided not to update the report “Exercise therapy for chronic fatigue syndrome” due to insufficient new research in the field and a lack of resources to oversee this work. They have been dragging their feet for five years now, and this is it?
Very disappointing news from Cochrane

@deevybee @GSCollins @cecilejanssens @f2harrell @BMJ_Open @meerpohl @liviapuljak Cumulatively combining good and bad evidence means you lose the information about what went wrong (and why) in individual studies. Sometimes all studies in a review are bad in the same way and that is used as a defence of poor methods. Like this disaster https://t.co/A5uDNifBSt
@Mr_NME @AnilvanderZee I am afraid that the Cochrane report is not withdrawn. It is being revised after some complaints. The new revision should be published any day now.

"Chronic Fatigue Syndrome (aka. myalgic encephalomyelitis) was intended as an example of the broader theme. We are awaiting a decision from the Cochrane Review about the paper to inform any editorial decision. Read the latest here. https://t.co/31JKsAxaVI "
@mrsdrgielen @folketinget @regeringDK @UlrichFredberg Substantive criticisms of Larun's review were made in 2016, and she responded with evasive non-answers:

What do you think of Larun's replies?
@cochranecollab still refuses to wiithdraw its review judging the PACE trial at low risk of bias in all but one domain. Therefore the review concludes that exercise is moderately effective and doesn't make symptoms worse. https://t.co/BigJGnumNP https://t.co/dQvvgNQyKX
The right of access to biomedical care for ME/CFS is a bioethical issue says Diane O'Leary. i.e. when psychosomatic research proceeds as if it is simply not possible that patients with ME/CFS are suffering from a biomedical disease, medical risks....

If you author a Cochrane review there is an assumption that you follow the protocol, and that if you don't, you report the changes in the review. The protocol is made inaccessible. What's that about @cochranecollab?? I can't find the protocol for https://t.co/RTOezDWYcZ https://t.co/INfw5Maq8O

@IdiotTracker @teresaboat @_Lucibee @richardvallee @RobertHMcMullen @ProfRobHoward @wilshica Thanks for following up on this.
Cochrane currently warns that this review is "substantially out of date and in need of updating." https://t.co/5WkjLs6kU1
The RCT's have been misleading because harms are poorly reported in nonpharmacological interventions.

@RichardBentall @D_Blanchflower I copied that from here (on the review you linked to): https://t.co/GCjzAX1JXF
It's also worth reading the comments from Courtney & Kindlon that explain some of the key problems with review. Larun failed to provide adequate responses, but this problem was ignored for some time.
@D_Blanchflower Bang up to date Cochrane review of trials (includes my own) that included exercise component (various control treatments). Compared to passive control (I guess you'd say this was the counterfactual) benefits for fatigue with no evidence of adverse effects: https://t.co/8Wjma4SYbi

@nicolacullum @cochranecollab Also, concerns only seemed to be taken seriously after many other academics started speaking out, eg: https://t.co/OVc6az0j0I When patients like Courtney first raised concerns they were dismissed with non-responses, eg: https://t.co/mqYZJRzVlB https://t.co/YF50cAxc5u
@CindyFarquharNZ eg: Patients have been pointing out problems with this review for years, but Cochrane has failed to take action: https://t.co/GCjzAX1JXF It has been used to promote misinformation, prejudices and support junk-science like the PACE trial. It is far from the 'best possible'.

Evidens?...but judge that further work is needed to ensure that the review meets the quality standards required, and as a result have not approved publication of the re‐submission. The review is also substantially out of date and in need of updating. https://t.co/2tVRARpod2
