Abstract in brief
Superior oblique myokymia (SOM) is a rare eye condition with an unclear cause, often affecting middle-aged individuals and usually more common on the right side, especially in women.
The condition is thought to result from neurovascular compression or abnormal nerve transmission, and various treatments like medications (including topical beta blockers such as levobunolol), surgery, and neurosurgery have been used, but no definitive cure exists.
Given its rarity, treatment approaches are still being explored, with topical levobunolol recommended as a first step before considering more invasive options like systemic medications or surgery.]
For @NeurosurgeryCNS, a team describes what they think is the first report of fully endoscopic microvascular decompression of the trochlear nerve in a patient with superior oblique myokymia.

Don’t think it really matters, honestly. I’ve been getting my eyes checked at LC for years and just recently went to a PV by my house and it was fine. And I have superior oblique myokymia, so I’m a tougher patient than average. Gotta go to an ophthalmologist every couple years.
@DrSSDeol168927 @PawanJaat0005 The stethoscope is particularly helpful in the neuro-ophthalmic evaluation in two situations: listening for vascular bruits (embolic and vascular occlusive diseases, and carotid cavernous fistula) and detecting the flutter of superior oblique myokymia.
