What people are saying
Many discussions highlighted the dangers and treatment protocols for toxic alcohols, with users referencing specific antidotes like fomepizole and the importance of urine and serum osmolality in diagnosis. Phrases like 'toxic alcohols really are very, very bad' and 'is one of the most interesting things to argue about in all of toxicology' illustrate a mix of alarm and fascination, emphasizing the language's urgency and importance in medical contexts.
Most discussions reflect support for the publication's insights, particularly regarding the recognition of toxic alcohols as serious health threats.
There is a strong curiosity among participants, as many express intrigue about the mechanisms and implications of toxic alcohol poisoning.
While some posts demonstrate deep engagement with clinical details and antidote information, others remain more superficial.
Participants widely acknowledge the significant health risks posed by toxic alcohols, indicating a high perception of their impact on medical practice.
Abstract in brief
- Toxic alcohols refer to substances like methanol, ethylene glycol, and isopropyl alcohol that can cause serious health issues when ingested.
- They can lead to symptoms such as metabolic acidosis, neurological impairment, and even death if not treated promptly.
- Immediate medical attention is crucial, as treatments like antidotes and supportive care can effectively mitigate their toxic effects.

Why does this matter? Toxic alcohols like methanol and ethylene glycol require prompt ID and treatment, and treatment can be invasive (like hemodialysis) and costly. The osmol gap is the single most used test for the purpose of identifying and starting treatment. And it is a really inaccurate test!

Fomepizole. https://t.co/kVu3mzHuRx Fomepizole is a crucial antidote for methanol and ethylene glycol poisoning. Ingestion of toxic alcohols such as methanol and ethylene glycol is uncommon, but not rare, in the UK and US. These substances are found in all sorts of household and industrial products, including antifreeze, paint strippers, coolants, and cosmetics.
Slow fermented honey vinegar is a thing. Which could make more toxic alcohols redundant for paint.

Much of it comes down to what would change clinical decision making? It feels like most of the time time what's the point? That said, I like having the volatiles by GC so we can differentiate the toxic alcohols.
@thepowerofozone Nobody doubts that. Ever heard of the Herxheimer reaction? But the study specifically uses allyl alcohol, which is a model compound that barely anyone is exposed to in such amounts. You‘d have to come up with similarly toxic alcohols/aldehydes that are native to the body
