Abstract in brief
- The study developed a machine learning test called PROphet that analyzes plasma proteomic profiles to better predict which metastatic NSCLC patients will benefit from PD-1/PD-L1 inhibitor therapies, addressing limitations of current guidelines that rely mainly on PDL1 levels.
- The trial, involving 540 patients on immunotherapy and 85 on chemotherapy across multiple centers, showed that PROphet accurately predicts clinical benefit, with high agreement between predictions and actual outcomes, and can further stratify patients beyond PDL1 expression levels.
- The results indicate that PROphet could guide more personalized treatment decisions: PROphet-negative patients with high PDL1 levels tend to do better with combination therapy, while PROphet-positive patients have similar outcomes whether treated with immunotherapy alone or combined with chemotherapy.]
